2004 POOL RULES ACKNOWLEDGEMENT FORM

Please familiarize yourself and discuss the pool rules with your household. If you submitted an acknowledgement form in 2003, that is still on file, you do not need to resubmit a form this year. If you did not turn in your form last year or are new to the area, please return this form as detailed below.

I have read and understand the HOA2 pool rules for 2004. If I have any questions or need further explanations regarding any item or subject covered in the posted HOA2 pool rules, I shall contact Cyneil Saxton, HOA2 Pool Committee Chair at 720-733-8378 or e-mail at rdsaxton@aol.com. In the event I become aware of a potential or actual rule violation(s), I shall immediately report the facts.
